Anyone whose personal information was accessed without authorization during the December 2024 Community Dental Care data breach, including people who received a personal notice about the data breach.
What’s This Class Action Settlement About?
The lawsuit claims Community Dental Care experienced a targeted cyberattack in December 2024. This resulted in certain individuals personal information to be accessed without authorization, this included names, health insurance information, dates of birth, medical information and Social Security numbers.
Who Qualifies for the Settlement?
If your personal information was involved in Community Dental Care’s December 2024 data breach, you may be eligible to file a claim. This included individuals who received a notice of the data breach.
How Much Money Can You Get?
Eligible Class Members can choose between two cash payment options:
- Up to $5,000 for documented, unreimbursed out-of-pocket losses caused by the data breach
- $50 alternative cash payment with no proof required
Class Members may also receive two years of medical and credit monitoring through CyEx Medical Shield Complete.
Is Proof Required?
Proof is required for the up to $5,000 out-of-pocket loss payment. Claimants must provide documentations such as a bank statement or receipts to support the eligible unreimbursed expenses. However, no proof is required for the $50 alternative cash payment.

- Claim Deadline: October 19, 2026
- Exclusion Deadline: September 24, 2026 (PASSED)
- Final Approval Hearing: November 23, 2026 at 11:00 a.m. CT
